Section 1 – Information about the person or business
you are giving information about.
In this Section we need to know the following details, if known, of the person
or business you wish to give information about. It is important that you do
not seek out further information, by yourself or through others, on behalf
of HM Revenue & Customs.
- Details of their names used, full or part, including any alias or nicknames.
Their address, if only the area is known please include this, also include
any contact telephone numbers they use or have access to.
- Their Date of Birth or approximate age.
- Brief description of the person. Please include any distinguishing features,
height, colour of hair etc.
- Type of tax fraud your information relates to. For example, not declaring
all income, not registered as self-employed, not deducting PAYE/National
Insurance from employees wages.
- Details of vehicles used or have access to – Information relating
to vehicle registration number(s) and part numbers. Also include vehicle
type, make, model and colour and where the vehicle is kept.
There is a separate box at the end of this section that you can use to include
any other information you may think is relevant in identifying the person
or business.
Section 2 – Information relating to their business
details
This section is divided into two sub groups. Section A is for giving information
relating to the persons if they are self employed and Section B if the person
is an employer.
- Business or employers names and address, full or part. Also the type of
business and how long they have been trading for.
- Details of advertising the either the business or employer. For example,
local press, flyers, posters etc.
Section 3 – Using the information
This section will help us to assess how we appropriately use the information.
Please provide as much information as you can.
- Your connection with the person or business. Please specify whether you
still see or deal with this person or business, or whether the contact was
a one-off.
- How did you obtain this information? Please specify how you obtained the
information, giving details on whether you were told in confidence, witnessed
it, acquired it through your work, from an overheard conversation, or is
the information based on your suspicions.
- Are there others that would know this information? Please specify whether
the information is known only to you, a few people or is widely known. This
will help us to understand and manage the information provided.
- Will it affect you or others if we use this information? Please specify
if it will affect you or others. If the answer is yes we need to know who
the others may be and how it will affect them.
- Are you prepared to repeat this information in a Court of Law? Please
specify if you are prepared to go to Court to repeat the information that
you have passed.
There is a separate box at the end of this section that you can use to include
any other information you may think is relevant.
Section 4 – Your details
This section relates to your own personal information.
If you are willing to be contacted to clarify the information further, we
will need your details which will include:
- Your full name.
- Your date of birth.
- Your address.
- A contact telephone number where we can safely call you from. We also
need details when this would this would be convenient to you.
Section 5 – Submitting the form
We need an email address to send an acknowledgement to. If you would prefer
not to receive an acknowledgement then we have provided an alternative. This
box is mandatory.
