Help with downloadable forms

All our forms and notes are in Portable Document Format (PDF). These can be viewed, downloaded and printed.

Some of our PDF forms can be completed on-screen then printed out. You will not be able to save the completed form.

To save copies of the completed forms to your hard disk, you will need to purchase the full version of Adobe Acrobat from Adobe and install it on your computer.

We recommend that you download the form to your computer, complete it on-screen and print it out. However, these forms can also be printed and completed by hand.

The form MLR100 Application for Registration has a continuation sheet - MLR100 (CS). This is for use with page 9 of the MLR100. The downloadable version has a reset button which will allow you to input the information, print the page and then clear the content by pressing the ‘reset’ button to allow you to add information for further trading premises.
