The employee's doctor may also be asked for medical information about the employee’s incapacity, using form SSP42, SMP42 or SSP45 as appropriate, at the time of referral to Medical Services.

The employee must give written consent before HMRC the case can be referred to Medical Services. If they have completed form SP14 requesting a HMRC decision, they will also have signed the necessary declaration.

If the employer asks for a medical referral, they should include the employee's written consent, in the form shown in the E14 Employer Helpbook for Statutory Sick Pay, with their application.

In all other cases ask the employee to complete and sign a form SP14, SSP/SMP44 or SMP56 .
