PS 80C - Certificate of Award of Initial Penalty

You should send four copies of this form, which is available only in paper form, to the Inspector. You should fill in only the administrator's name(s) and address(es). If a penalty is imposed, a completed copy will be returned to you for placing on the penalties sub-file.
