SELECT 4, CREATE AVR REMINDER

b. Input details from Part 1 of PS9019 and press ENTER. If Part 2 is completed, continue as shown below; if not, no further action is required.

SELECT 2, AMEND FILE

c. Input scheme reference number.

SELECT 3, ENTER AMENDMENTS

SELECT 2, MISC. AMENDMENTS

d. Check details against Part 2 of PS9019, amend as necessary, then press ENTER.

e. Sign and date the form then:

• if dealing with off-file, refer to nominated manager;
• if dealing with on-file, refer to file marking shown on form.
