type of scheme:

| Non-standard insured | - green pouched/unpouched (depending on volume of accompanying documents) |
| Individual arrangement | - manila folder |
| Hancock annuity | - manila folder |
| Self-administered | - green pouched |
| Unfunded | - green pouched |
| Participating employer (associated) | - green folder |
| Participating employer (unassociated) | - manila folder |
