Letters And Forms On Computer (Info)
Claimants’ personal details are only carried on the first page of most of the Letters and Forms for Computer (LFC).
Therefore, if the first page of an LFC is not returned you cannot identify the claimant. This results in a large amount of correspondence that cannot be linked to a work list item.
Space for personal details is already provided on some LFCs and will be added to others at a later date.
You must follow the guidance below every time you issue an LFC. This will help in identifying claimants and linking correspondence.
Make sure that every page of the LFC shows the
- Claimant's name
- Claimant’s NINO
- Operational Unit ( OU) number
And
- Management Unit ( MU) number
Note: If you are unable to enter this information onto the LFC before printing, then you must manually write it on the LFC after printing.
Form TC609
This LFC is issued with special blank claim form pages to obtain further information from claimants.
The blank claim form pages to be used with the form TC609 have a space for the
- Claimant's name
- Claimant's NINO
And space for
- The OU and MU numbers
You must use the specially designed blank claim form pages when issuing this LFC, making sure that the details at the top of each page are fully completed.
Note: Do not use photocopies of actual claim form pages.
