Invite Fresh Claim (AG)

To deal with a situation where a fresh claim is required, consider steps 1 - 6. The guide is presented as follows

1If the claimant has contacted you by telephone’
2If the identity of the caller is confirmed
  • Explain why you cannot make the change where appropriate
  • Explain why a new claim must be completed
  • Confirm the current address
  • Tell them that you will send a fresh claim form for completion
3Issue a new claim form to the claimant. Where this is due to death or family break up
  • Ask the claimant to attach a note to the back of the claim form asking for backdating to the day after the death / break up

4Write to the claimant and

  • Tell them that you will send a fresh claim form for completion
5Enclose a fresh claim form for completion
6Return the original request for the change (if it was in writing) to the claimant
