Notifying Child Benefit
Office Of A Deceased Child (AG)
To notify Child Benefit Office of a deceased
child /
young person, consider
steps 1 – 3 below.
| 1 | Access the form TC649A
template on your desktop and complete the following details
- Date of death of deceased child
Then
- Save the form TC649A on your desktop and
go to step 2

| 2 | Open the WAM referral by
clicking on the start button on your computer and select the [E -
FORM] option
- In both Taxpayer fields enter the
claimant's surname
- Select [Other] in the Work Area field
- Enter the claimant's
NINO in the NINO field
- Click [Attachments] in the top right hand
corner of the [E - FORM] box
- In the [Add Attachments to E - FORM] box
select [Look In Desktop]
- Locate the TC649A highlight it and click
[OK]
Note: The attachment will appear in the bottom
left hand corner of the E FORM template
Then

| 3 | Click on [TO] in the top
left hand corner of the E - FORM template
- Highlight the [All Offices] option
- Scroll down the list to the [Child Benefit
Office WAM]
And then
- Click on the [Send] button. This will
issue the E -FORM to Child Benefit Office
