Notifying Child Benefit Office Of A Deceased Child (AG)

To notify Child Benefit Office of a deceased child / young person, consider steps 1 – 3 below.

1Access the form TC649A template on your desktop and complete the following details
  • Claimant details
  • Name of deceased child
  • Date of death of deceased child

Then

  • Save the form TC649A on your desktop and go to step 2
2Open the WAM referral by clicking on the start button on your computer and select the [E - FORM] option
  • In both Taxpayer fields enter the claimant's surname
  • Select [Other] in the Work Area field
  • Enter the claimant's NINO in the NINO field
  • Click [Attachments] in the top right hand corner of the [E - FORM] box
  • In the [Add Attachments to E - FORM] box select [Look In Desktop]
  • Locate the TC649A highlight it and click [OK]

Note: The attachment will appear in the bottom left hand corner of the E FORM template

Then

  • Go to step 3
3Click on [TO] in the top left hand corner of the E - FORM template
  • Highlight the [All Offices] option
  • Scroll down the list to the [Child Benefit Office WAM]
  • Highlight your selection
  • Click on the [OK] button

And then

  • Click on the [Send] button. This will issue the E -FORM to Child Benefit Office
