Death Of A Child Reported (AG)

To deal with a notification of the death of a child or young person, consider steps 1 - 8 below. The guide is presented as follows

1If notification of the death of a child is received in
  • A Tax Credit Office (TCO), go to step 2
  • An Enquiry Centre, go to step 3
  • A Contact Centre, follow the call type process for Death of a Child in the IRCC Adviser Guide
  • The Department of Work and Pensions (DWP) or a Job Centre Plus (JCP) office (In Northern Ireland it will be the Department for Social Development (DSD) or a Social Security Office (SSO) ), go to step 6

If a claimant notifies the death of a child to

  • The Child Benefit Office (CBO)

Or

  • The Child Trust Fund Office (CTFO) they will
  • Deal with their aspects of the claim
  • Request relevant information required by the TCO

And

  • Refer the case directly to the TCO Priority 1 Team

2Complete a form TC648
  • Mark the top of the Form TC648 with 'Urgent Priority 1 Team'
  • Complete the form with the claimant's
  • Full name

And

  • The following details, if you hold them
  • Name of the child
  • Date of birth of child
  • Date of death of child

And

  • If the claimant incurred any childcare costs for the deceased child
  • The full name and address of the child care provider
  • The weekly amount they paid for the deceased child
  • The dates payments to the provider commenced and ceased
  • Send the TC648 and any accompanying correspondence marked 'Urgent Priority 1 Team' to the Priority 1 Team through the internal post

And then

  • Take no further action

3Explain to the claimant that their case will be dealt with by a specialist team in the TCO. Ask them to provide you with their full name, NINO, and
  • The name of the child
  • The date of birth
  • The date of death
  • If they have made a claim to Tax Credits

And

  • If the claimant incurred any childcare costs for the deceased child
  • The full name and address of the child care provider
  • The weekly amount they paid for the deceased child
  • The dates payments to the provider commenced and ceased

Then

  • Go to step 4
4Record all these details on a form TC648 along with a contact telephone number or address for the claimant and details of a convenient time that the Priority 1 Team can contact them
  • Tell the claimant that a member of the specialist team will only contact them if necessary whilst they are dealing with their case

And then

  • Go to step 5
5When the claimant has left the Enquiry Centre
  • Telephone the Priority 1 Team at TCO (This text has been withheld because of exemptions in the Freedom of Information Act 2000)
  • Give the TCO the claimant's information
  • Give the TCO the contact details for the claimant
  • Mark the front of the form TC648 with 'Priority 1 Team' in red and send it with any accompanying correspondence the claimant has given you to the TCO in a yellow polyope(This text has been withheld because of exemptions in the Freedom of Information Act 2000)
     
  • Use Function MAINTAIN HOUSEHOLD NOTES to record the action you have taken

And also

Follow the guidance in Maintain Household Notes

And then

  • Take no further action

6Explain to the claimant that their case will be dealt with by a specialist team in the TCO. Ask them to provide you with their full name, NINO, and
  • The name of the child
  • The date of birth
  • The date of death

And

  • Whether they incurred any childcare costs for the child prior to date of death

And then

  • Go to step 7
7Record all these details on a form TC648 along with a contact telephone number or address for the claimant and details of a convenient time that the Priority 1 Team can contact them
  • Tell the claimant that a member of the specialist team will only contact them if necessary whilst they are dealing with their case

And then

  • Go to step 8
8When the claimant has left the office
  • Telephone the Priority 1 Team at TCO (This text has been withheld because of exemptions in the Freedom of Information Act 2000)
     
  • Give the TCO your details and a contact number
  • Give the TCO the claimants information
  • Give the TCO the contact details for the claimant
  • Forward the details you have recorded clearly marked with 'Priority 1 Team' in red and send it with any accompanying correspondence the claimant has given you to the TCO following your normal postal instructions

And then

  • Take no further action
