For tax credit purposes an adult can only be included in one family at any one time.
An entry will be made on the ‘Verification Failures’
Work List if a claim is already on the computer

You must first establish whether the
verification
failure is a possible mismatch case and if it is, refer to the
latest guidance that will advise you of the action that needs to be
taken.
If the verification failure is not a possible mismatch case,
you must establish the reason for the failed claim and to do this
you will need to contact the claimant. You must
- Contact the claimant in the failed claim first
And then, if you are unable to resolve the failure
- Contact the claimant in the existing claim
Note: When contacting the claimant, you must state that you need further information to confirm the details already held on the computer. You must not discuss or mention the other claims when you contact the claimants unless you have established that they are the same person, otherwise you will be in breach of the Data Protection Act
To resolve the verification failure you must
- Confirm that the details held on the computer are correct
And
- Establish if the same person is in more than one family
If you find that
- The details held on the computer are incorrect. You must follow the guidance in Incorrect Personal Information Held On The Computer
- There are two different people using the same NINO. You must refer the verification failure to the Mismatch Team
Or
- They are the same person. You must establish why they are in more than one family and take appropriate action to make sure that the claimant is only included in one family

Possible reasons for the same claimant appearing in a current award as well as another claim include
- Family separation
- Possible mismatch case
- The claimant has submitted more than one claim in error
- The claimant has submitted a new claim to notify us of a change of circumstances, for example, a change of address
- A fraudulent claim has been submitted
- A claim has been logged more than once in error
- The claim is a polygamous marriage case
Note: A claim may fail verification for more than one reason. You must consider all of the failures at the same time and, as far as possible, only contact the claimant once. You must also take the appropriate action to resolve each verification failure as soon as sufficient information has been obtained to do so
