IHTM23041 - How to make formal
referrals: introduction to VOA 1 and VOA 2
The basic principles behind the design of these forms are
that
- you are not required to duplicate
information already contained (
IHTM23012) in the formIHT405
- you do not need to write in standard
information that commonly applies. Certain of the boxes in the top
half of the form should be completed only when appropriate
circumstances apply
- the VOA 1 (
IHTM23042) is a single sheet form
- the VOA 2 (
IHTM23043) is a two sheet form, with
the lower, carbon, copy being retained on file
The VOA’s response (
IHTM23141) will be on one of their own
forms – either the VO 1110 (final report (
IHTM23143)) or the VO 1120 (response
after initial appraisal (
IHTM23222)).
