The identity check is only necessary when one of the

following applies

  1. CIS4 and CIS6 certificate holders requiring a replacement card with a
  2. - Change of photograph

    - Change of individual’s name (i.e. not partnership or

    company name)

    - Change of signature

  3. CIS6 certificate holders requiring a renewal certificate where there has been a noticeable change in
  4. - The subcontractors appearance

    - The subcontractors signature (possibly due to a change of name)

  5. The case is referred to you by BS CIS for an identity check

Note: if the circumstances in either 1. or 2. apply, both a photograph and signature must

be produced for scanning.

The applicant must bring their current CIS4 or CIS6 as proof of identity.

Photograph

- Check that the photograph is clear, and not too dark or too light.

- Check that it is easily recognisable as the applicant

- Check that the photograph now presented and that on the old CIS6 are recognisably the same person.

- In the photograph, the applicant must not be wearing a cap or hat, or dark glasses.

- Check that the photograph is passport size.

If any of the above are not satisfactory, you cannot accept the form and the applicant should be asked to supply a suitable photograph.

Signature (unchanged)

- Ensure that the signature is that of the applicant(i.e. check with old CIS6)

- Ensure the signature is within the box as instructed. If it is not, it will

not be able to be scanned and the applicant will have to complete a new form.

Changed Name and/or Signature

- Applicant must provide identification already showing the new name and signature.

You must not accept a change of name or signature without documentary evidence.

Examples of acceptable evidence:

- Marriage certificate

- Driving licence

- Passport

- Deed Poll certificate

If one of these cannot be produced, the form cannot be accepted. Tell the customer to return at a later date with acceptable proof of change of name and/or signature.

Changed Company Secretary/Nominated Partner (CIS5/CIS5(Partner) only)

Applicant must provide documentary evidence that they are the new company secretary or Nominated Partner. Acceptable proof would be a letter from the company/partnership on headed paper. This evidence may be supplied by post or be already present on the file.

What to do next

Renewals only:

- Check that entries have been made for at least one Turnover Test.

- Ensure that a Statement of Evidence has been provided with any application

based on the 6-month Turnover Test.

Forms CIS82(Individual) and CIS83(New) only: Enter the sole trader’s, partner’s or director’s UTR in the green boxes provided at the top of page 2.

All forms

- Ensure that the Declaration has been signed and dated.

- Tick the boxes saying that you are satisfied with the new photograph, signature, or change of name and signature

- Date stamp the form and sign over the date stamp.

Where to send the form

CIS27a:

With change of photograph or signature (but no change of subcontractor or concern details - 'Acting For' or 'Trading As' name): following the ID check, send the form direct to the CIS Centre for scanning.

With a change to subcontractor/concern details - send the form to the concerns SA/CTSA district to amend the SA/CTSA record. The form only needs to be scanned at the CIS Centre if there is also a change to the photograph and/or signature.

CIS27b:

All forms to be sent to the company CTSA or partnership SA district to make any changes to the CTSA/SA record for the concern. Forms including a change in company secretary or nominated partner signature must be sent to the CIS Centre for scanning.

Remember that no ID check of the company secretary/nominated partner signature is needed but evidence of the change is required in the form of correspondence from the concern or their accountant.

CIS82(Individual):

Send the form to the applicant’s SA District instructing them to change the SA record, and then to forward the form(pages 1 & 2 only) to the CIS Centre for scanning.

CIS83(New)for a partner (plus CIS84(Partner)):

Send the form to the Partnership’s SA District instructing them to make any changes to the SA record, and then to forward the forms (CIS83(New)plus pages 1 & 2 of CIS84(Partner) to the CIS Centre for scanning.

CIS83(New)for a director (plus CIS85(Company)):

Send the form to the Company’s COTAX district instructing them to make any changes the SA record, and then to forward the forms(CIS83(New) plus pages 1 & 2 of CIS85(Company)) to the CIS Centre for scanning.
