The Payslip

At the same time as preparing the letter of acceptance you should prepare a payslip using form 94A. If this is to be typed you need to complete a 94A(WS). The form 94A is in three parts with one copy for your papers, one copy for the recovery office (Accounting and Payments Service) and one copy for the claimant. Take care to ensure that the entries are made in accordance with Part (B) of the form 94NTC on which you calculated the suggested offer.

The form 94A requires a consecutive number for accounting purposes which you obtain from your Claimant Compliance Centre Manager (CCCM), who should also sign the form 94A. More instructions regarding the payslip follow in CCM8660.
