The text for a formal S18(10) notice is:

"Notice requiring information or evidence

Section 18 Tax Credits Act 2002

I wrote to you on [date] to ask for further information or evidence for the purposes of deciding whether you were entitled to tax credit, and if so in what amount, for [2003/04].

To date I have not received the items listed below. I am now giving you notice that you are required by law to provide the information or evidence listed below by [enter specific date - at least 40 days ahead]. If you do not, you may be liable to a penalty.
