IHTM03203 - Form 403: what is not completed on Form 403?

You do not normally need to complete the following information on Form 403

• Cross-reference to PN folder (IHTM03190) (item 1)
• Stats, O(Insts) - these boxes are not completed but are crossed through when the sub-file is closed
• Record of assessments
