Application Form

To reserve a place on one of the CATA HM Revenue and Customs courses (CTIC or AMP), please complete and send the following form. If you are unable to send the form using the button below, please complete it, print it and send it to me, Sean Rabbett, at the following address:

Your application must arrive here by 17 April 2009 at the latest.

It is important that your application is supported by your organisation and you should enter the name of the manager who supports your application. That person will need to confirm that your organisation has secured funding for your participation by 15 May 2009 or your place cannot be guaranteed.

You should receive confirmation of your reservation within 14 days. If you do not, please contact me at the above address as soon as possible.
