Completing the returns

Monthly return

Local authorities and Health and Social Services Trusts are required to submit the prescribed information.

The return can be made in one of the following ways:

• on the CTF15 (PDF 40K) and CTF15 (Child) (PDF 42K) - to use this form you need to print it out and complete the information by hand
• submitting your own template – as long as you provide the necessary information
