Completing the returns
Monthly return
Local authorities and Health and Social Services Trusts are required to submit the prescribed information.
The return can be made either:
on the CTF15 (PDF 40K) and CTF15 (Child) (PDF 35K). To use this form you need to print it out and complete the information by hand;
OR
using the guidance (PDF 72K) to type up the information. The guidance sets out the information that is required and the precise format that it must be submitted in. It is intended to allow those authorities/Trusts that wish to, to develop their own template for collating and submitting the required information.
